Assist – Personal Activities

A program that assists with and/or supervises personal tasks of daily life to enable the client to live as autonomously as possible. These individual supports can be provided in a range of environments, including but not limited to, the client’s own home. This service is achieved through the assistance of a Support Worker.

Assist – Complex Needs

A program that assists with and/or supervises personal tasks of daily life to enable the client to live as autonomously as possible. These supports are provided by a worker with additional qualifications and experience relevant to the clients’ complex needs. These individual supports can be provided in a range of environments, including but not limited to, the client’s own home. This will be achieved through the services of a Support Worker who has relevant specialist skill sets and experience, such as behaviour support, suctioning etc.

Assist – Life Stage, Transition

Providing assistance to clients for their entire life until the end. They have experienced nurses that have experience in palliative care and have many nursing skills.

Assist – Travel / Transport

Grevillea Care Services has its own transport vehicle for many passengers or for individual use. This will be achieved through the assistance of a taxi/bus driver.

Daily Task / Shared Living

The development of daily living and life skills program at Grevillea Care focuses on the training and development of activities undertaken by the client or their carer to increase their ability to live as autonomously as possible, including supports that will enhance the ability of the client. This service is achieved through the assistance of a Support Worker.

Behaviour Support

At Grevillea Care, we have a team of Behaviour Support Specialists who formulate, implement and monitor behaviour support plans that are timely, efficient and effective ensuring they meet the client goals. Our Behaviour Support Specialists are responsible for assessing, designing, evaluating and monitoring behaviour support plans for individuals with disabilities and have a comprehensive knowledge of their requirements working as a behaviour support specialist.

At Grevillea Care, we work within the guidelines of the NDIS Quality and Safeguarding Commission requirements, the NDIA Terms of Business, the Victorian regulatory requirements and any State Based Restricted Practice Authorisation requirements. It is important to us that we provide the best service to our clients and that is why we have carefully curated a team of specialists that each have Tertiary Qualifications specialising in Disability, Psychology, Social Work and Counselling. Our team of specialists ensure they work with the most up-to-date literature whilst providing evidence-based treatment, clinical knowledge and expertise in assisting clients with behaviour support.

At Grevillea care, we aim to improve the quality of life for people with disability and their families, and assure that we do this through the provision of our Behaviour Support service.

Group / Centre Activities

Assist clients to participate in group based community, social and recreational activities. This will be achieved through the assistance of a Support Worker.

Community Nursing Care

Assist with the provision of specialist care for clients who have high care needs requiring a high level of skill and for the training of support workers to respond to the participant’s complex needs. This will be achieved through the assistance of a registered nurse who is experienced in professional/clinician supervision and development as defined by the professional registration requirements of the relevant profession.

Household Tasks & Maintenance

Grevillea Care Services can assist clients with assistance with essential tasks that the client is not able to undertake such as house or yard maintenance, cleaning and linen. This will be achieved through the assistance of a cleaner, gardener and other.

Life Skills Development

Assist clients to train/learn new skills. This can be in the home or externally through formal training. The Support Worker assists by being there for support and advice.

Community Participation

A program that assists clients to participate actively in community, social and civic activities; includes supporting clients during these activities and developing clients’ ability to partake in these activities. This will be achieved through the assistance of a Support Worker.
